Controlled Substance Prescribing Handbook
This combines all of our controlled substance prescribing policies and education material into one document.  It includes the guidelines for pain, anxiety and sleep, and ADHD which are listed separately below.

Controlled Substances Agreement
This is the agreement we use for all of our controlled substance prescribing.  It includes several sections:  Controlled Substances Contract, Side Effect Forms, Pharmacy Selection Form, and Treatment Plan.

Drug Testing Information
This gives some brief information about drug testing.

Opioid Risk Tool
This is a brief questionnaire that helps identify patients who are at a high likelihood of abusing controlled substances.
